Evaluation & first steps

Start with your story.
Make room for the full picture.

A useful evaluation connects what you are experiencing with your health, history, circumstances, and hopes for care.

We take time to understand the question you are bringing. That may be a new concern, a difficult experience with treatment, or a wish for a more connected plan. We discuss recommendations with you so you can consider the next step with clarity.

A clear path

A conversation. An evaluation. A plan to consider.

01

A complimentary 15-minute chat

A first conversation with Hamilton Gaiani, MD, or with Eric O’Connor, DNP, APRN if you are interested in care with him, helps us understand what you are seeking and whether a full consultation and evaluation may be appropriate. It is an introductory, informational chat, separate from diagnosis or treatment.

You can ask about our approach, fees, and how the team works. A chat does not establish ongoing care or guarantee a prescription.

02

Intake with Eric O’Connor, DNP, APRN

New patients begin with a medical and psychiatric intake evaluation with Eric O'Connor, DNP, APRN. This brings psychiatric assessment and medical context together. We consider symptoms alongside health, sleep, medications, substance use, daily function, and the care you already receive.

The aim is an individualized diagnostic understanding and thoughtful recommendations, with room for your questions and preferences.

03

Ongoing care with your clinician

After the intake evaluation with Eric O’Connor, ongoing care can be with Hamilton Gaiani, MD or Eric O’Connor, DNP, APRN, depending on your preference, clinical fit, and availability. Recommendations may also include coordination with your existing clinicians, referrals, or a different level of care. If you are invited to join the practice, this is also your opportunity to decide whether the team is the right fit.

If we do not move forward together, we discuss what may be helpful and information about alternative care options based on the evaluation.

When you need a second look

A focused question deserves a focused evaluation.

A medication–psychiatric consultation can help clarify a complex or unsatisfying treatment plan.

Depending on the question, this may include diagnostic clarification, a review of medications and interactions, discussion of benefits and risks, or recommendations to share with your treating clinician when authorized. The scope and fee for focused evaluations or written work are confirmed in advance.
