- Introductory 15-minute chat: no fee.
- Initial evaluation with Eric O’Connor, DNP, APRN: $250.
- 30-minute follow-up with Hamilton Gaiani, MD: $250.
- 30-minute follow-up with Eric O’Connor, DNP, APRN: $190.

A Good Faith Estimate puts the expected services and charges for your care in writing, using the applicable visit rates. It helps you plan before committing to care.
If you do not have health insurance or are not using it to pay for care, you generally have the right to a written Good Faith Estimate of expected charges before scheduled, non-emergency care.
When to expect your estimate
You can request an estimate before scheduling. If you schedule care at least three business days in advance, your provider must generally give you an estimate:
- Within one business day after scheduling care 3–9 business days ahead.
- Within three business days after scheduling care 10 or more business days ahead.
- Within three business days after you request an estimate before scheduling.
What the estimate means
The estimate should describe the expected items and services and their expected charges. If the recommended services change, we discuss the plan and associated fees with you. An estimate is not a contract requiring you to receive care.
If a provider's bill is at least $400 more than that provider's Good Faith Estimate, you may be able to dispute the bill through the federal patient-provider dispute process. Keep a copy of your estimate and bill.
